Dream Up an Event All Your Own!
We request that you help us by first downloading, filling in, and returning the application below. Please complete this at least four to six weeks before your event. Doing so allows us to verify (should inquiries be made) that you are indeed an approved GCHAS fundraiser; it also gives us time to consider promoting your event on our fundraiser calendar, and potentially, in our Strong Kids Newsletter. Once completed, please fax your form to (585) 273-5114 or e-mail it to keisenberg@admin.rochester.edu (please help us by placing “Fundraiser Application” in the subject line). We’ll be sure to respond with approval within a week after we receive your application. At that point, we’ll also be able to give you any pointers or guidelines that might be helpful. We can also answer any questions you might have. Finally, given the volume of community fundraising events each year, we apologize in advance that it may not be possible for a hospital representative attend. Please know that your efforts are still important to us! We gladly accept photos from your event, as we often post them online or highlight them in our newsletter to celebrate your creative work!
